Greetings Fellow Chiros and Happy Friday the 13^th!

I enjoyed reading an article that Dr. Shelley Simon wrote in the Oct. 21, 2009 issue of Dynamic Chiropractic called Patient Stories: Negotiating Shared Clinical Reality to Improve Outcomes to read in its entirety, see link below: (http://www.dynamicchiropractic.com/mpacms/dc/article.php?id=54273

I know that when I am with a new patient, it is almost impossible for me to resist the urge to interrupt them to share my years of experience, but Dr. Simon cautions against this. What I try to do now, when sitting down with a new patient who has filled out the New Patient Intake form: http://www.eznotesinc.com/docs/patient_intake_form.pdf , is to look at the pain drawing and simply say, What is the story that goes with this picture?

Then heeding the expression that states Since the body has 2 ears and 1 mouth, it is a good idea to listen twice as much as you speak. I let them talk (for a while anyway). Not only do I hear how they think their problem began, but what their belief systems are about health and illness. You will also remember that the question on the New Patient Intake Form #14 asks, What concerns you most about this problem, and what is it preventing you from doing? Questions like these help to learn their story.

This is exactly what Dr. Simon suggest that we do, when she writes:

The patient owns the story. Our job is to help patients make sense of deeply embedded and/or conflicting stories and craft a new story that will motivate them toward more positive outcomes.

For example, at the urging of a spouse or family physician, a patient sees an orthopedic surgeon who recommends surgery and physical therapy. Although the patient had been making steady progress under your care, he abruptly discontinues treatment in favor of following the new story line. What do you do in a situation like this? You could call the orthopedic surgeon, challenge the patient's story, or reignite your existing story about conventional medicine. Or you could calmly work with the patient to co-create yet another story, one that might actually include chiropractic care for a mutually agreed-upon time frame to determine if surgery could be avoided.

I think you get the idea. The software and your listening skills will help discover the patients belief systems and then it is your job to reframe them for a positive outcome.

Postural analysis:

If you down loaded version 09.23 (Nov. 6, 2009), you saw that Russ animated the posture figure that appears in the Ortho section of Regular Visit and in the Exam section under Posture.

Now, when you indicate that the patients left ear is low, the head tilts. There are about 4 other areas that will move. (If you want Russ to add the feet, i.e. Pronation and supination, write him, he would love the feedback)

One of our doctors mentioned that if you place tape on a door to make a grid, and used that as a background when taking a photo of the patient to add as a default image http://eznotes.websitetoolbox.com/post?id=3540435 then you could easily show the patient his/her distortion patterns and exaggerate them using the posture figure. It also would make a good mall screening tool similar to using a SAM (Spinal Analysis Machine). Give it a try!

Greetings fellow Chiropractors;

I am pleased to announce the latest version of E-Z Notes (V: 09.23) is ready to be downloaded. As always, please keep your suggestions coming, and I will continue to do my best to implement them as quickly as I can. Below is what to expect with this update:

1. The figure in Posture screen in Regular Visit and Exam will now change as you make your choices. This means, that if you indicate that a patient has a low right shoulder, the figures right shoulder will drop as a demonstration to the patient. This may be great for screenings!

2. Patient names coming from the Kiosk will now be timed stamped like they are in Limbo.

3. A few misspelling have been found and corrected.

4. Some knee options for Kinesio Taping have been added.

5. Work was done on Complete re-exam in an attempt to include the verbiage from the any test you added as well as to ensure that some of fields do not get drawn over other fields when more tests are added.

6. A button, See Past DXs has been added to the DX screens in case you would like to see a patients history of DXs before assigning a new one.

7. You can now access the 3 custom yellow buttons in Alleviates the patients complaint in New Patient through Set Up and Edit New patient Settings.

8. Work was done in an effort to help clean up any errors your staff may have done when entering in patients names with invalid characters inside your billing software when you are integrated.

9. The Kiosk computer will now have a button for numbers the patients can use if their password has numbers in it.

10. The images and quotes have been added to the Kiosk computer.

Dear Fellow Chiropractors:

My last couple of newsletters dealt with the writing of customized text for your SOAP Note (http://eznotes.websitetoolbox.com/?forum=36645 ). I have since learned from Russ that we can use the apostrophe () with the XXX to make the sentence even better. This is how it is done. For example, I used the Russian SCENAR device (http://www.energetic-medicine.net/scenar.html )on XXXPatientFirstNameXXXs problem area for 5 minutes to accelerate XXXhisherXXX healing time. Note the s, this makes the sentence a bit more normal and it will look dictated without the cost of transcription. In summary, write any sentence with the patients name and the appropriate pronouns i.e. his, her, he, she. Then add the XXXs substituting XXXPatientFirstNameXXX for their first name and XXXhisherXXX and XXXhesheXXX when called for. If you normally do a therapy for a specific amount of time, you can indicate that. Yes, this looks strange, but it makes your notes unique and will save you time.

Correcting Diagnosis Codes:

ICD-9 codes come and go and we will let you know if you have to change them in the E-Z Notes software. You might think we could do this automatically with our updates, but when Russ opened up that part of the program to customization, it prevented us from making the changes without erasing any codes that you added yourself. Therefore, it has to be done by each individual doctor.

I was alerted by Julie at Doctors Access that the code 715.5, which we have listed as Osteoarthritis- Hip is a bad code and does not exist as such, it should be 715.15 ( http://en.wikipedia.org/wiki/List_of_ICD-9_codes_710-739:_diseases_of_the_musculoskeletal_system_and_co nnective_tissue) .

To go into the software and remove it or add the 1 , this is what you do: Go to Set up, enter your password (eznotes, if you have not changed it) select Personalize your DX Codes and delete the code 715.5 or make it 715.15 in any section it exists, if it ends up on a CMS 1500 form, it will get bounced back without payment.

The other code that is a problem is the 719.5, it is missing the fifth digit (0-9)

The following 7 locations are here I found the 719.5: box 4 of Lumbar, box 4 of lumbar with extremity, box3 and 4 of Cervical, box 4 of Cervical with extremity, box 4 of Thoracic, Multiple Regions in miscellaneous section, and Entire DX list. To delete it, Highlight the code and hit delete. The first time you hit delete, the code goes away, the second time you hit delete, the space you created goes away. Remember to hit Save at each screen. With a little practice, you will get good at this.

I am pleased to announce the release of the latest version of EZNotes (V:09.22). This update has a few very large features that I am sure almost everyone will benefit from. As always, Keep the suggestions coming and I will continue to do my best to implement them as quickly as I can. Below is what to expect with this update:

1. A Modify option has now been created for the Gonstead, Proadjuster, and Pulstar techniques in Regular Visit.

2. An issue with the Other button in the new exam palpations and observations screens has been corrected.

3. The integration with Inphase is now complete. You can integrate by pointing your DX codes or sending them all together.

4. The Pierce technique has been added to the Regular Visit screens with a Modify button.

5. The integration testing with Inphase is complete. Please call to schedule a time for integration. Please note, we are doing this as a tag team with Inphase. They will do the first part and then we will finish it.

6. The Instruction Manual has been updated to reflect some of the additions to EZNotes.

7. Patients UIN and DOB data will now be shown in Treatment Plans and Narrative Reports.

8. The text randomization has been opened for the Pierce technique in Regular Visit.

9. An issue with the second Create your own Exam Screen in Shoulder not be available in your Personal Exam Sequence has been corrected.

Windows 7 ??

Has anyone tried to run EZ Notes with Windows 7? Is it compatible? Did you have any problems after the upgrade?

Our IT Consultant says it is by far superior to any previous OS by Microsoft but I wanted to know how it would play with EZ Notes. Any help or comments would be appreciated.

Dear Fellow Chiropractors:

If you read the Last News to Use #122 about creating custom text, http://eznotes.websitetoolbox.com/post?id=3690578 , you saw that you can generate full sentences that describe whatever you want selecting words that are CPT correct. In this issue, what I want to go over the ability to have the note automatically select the correct gender pronoun, as well as the patients first name. These tricks help make the note look dictated. For example, you could write that you placed the patient (no-gender specificity) in a Hubbard Tank (http://dictionary.reference.com/browse/hubbard+tank) , or you could say, that you placed him or her (gender specific) in the Hubbard Tank.

The way you would write it with the Yellow button is like this: I placed XXXhisherXXX {note the Xs are uppercase (capital letters)} in the Hubbard tank. The software will know based on the gender of the patient, which pronoun to use.

Another cool way to vary the text is to use the patients first name. In this case, you use: XXXPatientFirstNameXXX. For example, I was listening to the D.C. Tracts (a company that provides CME credits: 800.342.0454 http://www.datatrace.com ) Fall 2009 issue about mechanically induced pelvic pain and organic dysfunction. They show an AP lumbopelvic radiograph with a uterine fibroid, demonstrating a classic popcorn calcification pattern.

If I had a practice where I saw a lot of these x-rays, I could create a yellow button in the x-ray section for abnormal findings that might say: The lumbar AP film demonstrates that XXXPatientFirstNameXXX has the classic popcorn calcification pattern seen with uterine fibroids. I have advised her (the gender here would be obvious, no need for the XXXhisherXXX) to speak with her gynecologist. Anyway, I am sure you get the idea, if you find a problem with the text; you can always fix it and modify it at a later time.

Can we reverse the upgrade to the patient reports, in the past when we clicked on a dated and viewed that note, when we returned that note remained highlighted and the list stayed in the same position. I know it doesn't seem like much but before I send out any records I proof all notes because I have 3 other docs, and sometimes there may be a mistake. it slows the progress down since you removed it from the program.

I am pleased to announce the latest version of EZNotes (09.21) is ready to be downloaded. This update has some very useful new features. I think you will all appreciate item number 7 below, and I will do my best to get this as option for all of the adjusting techniques. As always, keep the suggestions coming, and I will do my best to implement them as quickly as I can. Below is what to expect with this update:

1. On the Palpations and Observations screens of Regular Visit, Exam, and PART Exam, when you place your mouse over the Import (all) button EZNotes will display for you what you would be importing.

2. An Import (choose) button had been added to the Palpations and Observations screens of Regular Visit, Exam, and PART Exam. This button will let you decide which categories you want import in the event everything wasnt the same or something was not evaluated.

3. An Undo button was added to the Capsular Swelling screen in Regular Visit. If you ever find a screen that you want an Undo button, please let me know.

4. An issue with Re-exam and the last Improvement button you select not be counted has been resolved.

5. An issue with the Muscle Weakness data not showing up in the last Palpations and Observations screen has been corrected.

6. An issue with the Custom Prognosis button 2 not replacing the old on in Regular Visit has been identified and corrected.

7. You can now Modify your segments when using Diversified, Drop Table and Arthro Stim in Regular Visit. This new feature will enable you to just change one or two segments without needing to remember which ones you selected previously.

8. The Kiosk has been updated with new images and Quotes.

9. The Instruction Manual has been updated to reflect some of the new changes with EZNotes.

10. You can redo a Case History for a specific case that a patient already has.

Re-Exam PART

What's the best way to re-exam a PART? Using the re-exam buttons generates a regular report, not in PART format. Using the PART button doesn't allow for changes of the patient's complaint areas. Thanks for the help!

Reporting without using VAS

I think it would be a good idea to be able to select each problem just like we do now but be able to report much better, slightly better, better,same, worse, slightly worse, much worse for each complaint or numbers if we choose. Not all of my patients report using VAS. If these options were in the VAS screen along with numbers it would be another option to report on each complaint. Reporting a VAS number on the first visit would establish a baseline. Then reporting better, same, worse etc... would suffice for each complaint. It might read like this...Mrs. Jones reports her headaches are better today. On a scale of 0-10 her initial headaches were an 8. Something like that.

News to Use #122: Choosing the text for your Yellow Buttons

Greetings Fellow Chiropractors:

A few weeks back, Russ and I exhibited at the Florida Chiropractic Association show in Orlando. A few booths away from us there was a gentleman selling a unique back support, called Back RX. I tried it, loved it and bought a bunch of them to sell in my office. It occurred to me that there are certain therapies like Ultrasound that can only be billed with a one specific CPT code like, 97035 (in the case of ultrasound), while others are more open to interpretation (the proverbial Gray area) .

I began composing what a Yellow button might say for the Back RX if it were billed under: 97110 (Therapeutic Exercise), 97112 (Neuromuscular Reeducation), and 97537 Activities of Daily living-Work). Truly, any of these codes could be considered appropriate, and possibly the determining factor for which one you would use, is which gets you paid better, as well as scope of practice in your state.

Below are the samples that I wrote with the CPT code at the end:

I instructed the patient how to use the "Back RX". This is a device that looks like a lumbar support, but is made of elastic material and provides resistance when doing a pelvic tilt. Since the patient sits at a desk all day, it is my goal to help develop low back extensor musculature as well as core stabilization strength. These exercises will improve range of motion and flexibility, and if done at home, as I suggest, will help with endurance and long hours of sitting. (97110)

I worked with the patient using a "Back RX" (elastic lumbar support). This piece of equipment, which is worn around the waste and low back, aids in proprioception for sitting. By rocking back and forth with the elastic resistance, the patient will develop an increased kinesthetic sense which will help with posture, especially at work. Since the patient is seated many hours a day at a desk, this device can be used while on the job to minimize low back extensor muscle wasting and will help in a "work hardening" sense, to keep the body strong. I trained the patient on how to use a "Back RX" (elastic support). This device is an assistive technology specifically for the patient's office desk chair. It is an adaptive piece of equipment to help with poor posture, which I believe is one of the causative factors of the patients low back pain. If the patient can improve the work environment, by using the Back RX on the job, there is a greater chance of a long lasting improvement and a reduced need for palliative care. (97537)

As you may have noticed, I borrowed heavily from the CPT book peppering my text with the buzz works that the insurance company adjustors like to see.

Another example of customizing a Yellow button for a therapy that E-Z Notes does not have in the modalities section, would be for IMPACs VibraCussor (http://www.impacinc.net). I was looking through D.C Products Review and I am plagiarizing what they wrote below:

I used the IMPAC 12 speed VibraCussor on the patients problem area for 5 minutes. This instrument delivers percussive impulses deep into the tissues of the body, and the repetitive pumping action helps release fascial restrictions, treating trigger points, relaxing muscles and releasing tight joints. (97140-59)

I think you get the idea. You can also use the E-Z Notes feature Visit Reminders (which you can check out in the Instruction Manual on the start screen), to remind yourself to show a patient the Back RX on visit #5 for example, in conjunction with other ergonomic tools.
